3 steps to penis lengthening and thickening/girth enhancement
Anesthesia
Surgical penis enlargement or penis thickening with autologous fat is always performed under anesthesia. This is tailored to your personal well-being by our experienced anesthetist so that you can go home on the same day.
Penis enlargement method
Your penis consists of an external part that is visible to you and a part that lies inside your body. The ratio of inside to outside is approx. 50:50.
Penis enlargement involves moving the inner part of the penis to the outside.
To do this, an angled incision is made at the base of the penis in the pubic hair area and the front retaining ligaments of the penis are detached from the pelvic bone. The previously curved and sloping inner part of the penis is straightened, thus lengthening the visible outer part of the penis.
Future stability is ensured by filling gaps with autologous tissue at the site of the lengthening, by leaving the posterior retaining ligaments in place and by using a special suturing technique.
The now externally visible and elongated part of the penis is covered with the existing skin of the penis and the pubic mound. This results in a Y-shaped scar.
Achievable length gain
Achievable penis lengthening: how many centimeters a penis can be lengthened depends on the existing inner penis length, the length and tightness of the connecting ligament to the pelvic bone and the condition of the ligament. Depending on the anatomical conditions, a length gain of around 2 - 4 cm can be achieved in most cases, in favorable cases even 5 cm. Rarely only a small amount of length can be gained due to anatomical preconditions. Unfortunately, this cannot be predicted or determined preoperatively.
Method of penis thickening / girth enhancement
Based on my experience and success, I only use autologous fat to thicken the shaft of the penis:
For this purpose, autologous fat is removed in the form of liposuction, e.g. from the abdomen or thighs, specially processed and prepared for retransger to the body. The autologous fat is then inserted under the skin of the penis using a special technique using blunt cannulas.
Not all of the fat actually grows in. Around 50% - 70% of the autologous fat grows in and remains permanently. How much fat actually grows also depends on physical factors and cannot be guaranteed in individual cases.
Men are usually very satisfied with the result immediately after the operation and want to maintain this condition as well as possible. In these cases, a second surgical session may be useful to achieve the desired result.
Aftercare and recovery
HEALING after penile thickening/girth enhancement: The healing process takes about 3 months in total. Immediately after the operation, a specially wrapped bandage is applied, which must be worn for 4 weeks. After 4 days, the bandage can be removed from the penis for the first time and light massages of the penis can be performed. You will receive special instructions for this after the operation.
During the healing phase, a "rest period" of at least 6 weeks must be observed.
This applies to sexual intercourse as well as masturbation and anything that causes tension, friction or pressure.
You will be fit for work again after around seven days and light sporting activity is permitted after around six weeks. After the healing phase of 6-8 weeks, regular postoperative stretching exercises must be performed so that the penis does not retract again. Special vacuum pump systems are available for this purpose and we will be happy to advise you.

What is a penis enlargement?
Penis lengthening is the surgical displacement of the inner part of the penis to the outside in order to achieve a lengthening of the penis by 2-5 cm in most cases.
What is penis thickening / girth enhancement?
Penis thickening with autologous fat can increase the girth of a thin penis by up to 20%. Penis thickening is usually performed at the same time as penis enlargement. The patient’s own fat is used as a filling material, as it does not cuae any defense reactions and around 50%-70% of the fat grows in permanently.
What are the reasons for penis enlargement?
- You are bothered by the length and size of your penis
- Your sex life is affected by the length and size of your penis
- If you are ashamed of the length of your penis and your self-confidence and quality of life suffer as a result
What happens during penis lengthening?
Half of your penis is visible to you outside your body and the other half is inside your body. The penis is fixed to the pelvic bone inside the body with so-called anterior and posterior retaining ligaments.
During surgical lengthening of the penis, the anterior retaining ligaments are released, the part of the penis inside the body is pulled outwards and the anterior retaining ligaments are reattached using a special suturing technique.
Any gaps are filled with the patient’s own tissue and the “new”, now visible part of the penis is covered with the patient’s own skin. The part of the penis that was previously inside the body is also straightened by the operation, which is part of the lengthening.
The condition of the ligaments and the length of the connecting cartilage to the pelvic bone are also decisive for the lengthening. At the end of the operation, a bandage is applied, which must be worn as prescribed in order to achieve a beautiful result.
Why is penis thickening (girth enhancement) performed?
- Because the girth of your own penis is perceived as too thin
- Because the condition of the penis affects self-esteem and self-confidence
- Because the sex life is not satisfying due to the current size of your penis, possibly resulting in erectile dysfunction
- You are restricted in your quality of life by a penis that is too thin
What happens during penis thickening?
Penis thickening is performed in two steps:
- Autologous fat removal from the abdomen or thigh, for example. The autologous fat is processed and specially prepared for transfer to the penis shaft.
- Transferring of autologous fat into the penis shaft: the specially prepared autologous fat is transfered under the skin of the penis shaft using a special technique with the help of microcannulas and distributed evenly. Since not all of the autologous fat injected remains permanently, a little more as needed is injected.
Approximately 50%-70% of the fat igrows in over the next weeks and months and remains permanently. Immediately after the operation I will apply a special pressure bandage, which you should wear for 4 weeks.
Penis thickening is usually performed in combination with penis lengthening, but can also be performed as a separate procedure.
Who performs penis enlargement procedures?
Penis enlargement and penile shaft thickening with autologous fat should only be performed by experienced plastic surgeons, as it requires knowledge and experience to fatgraft in such a way as to ensure a permanent and even result.
What if my penis is too short and too thin?
If you are affected by both the length and girth of your penis, a combined procedure of penis lengthening and penis thickening can be performed.
Can penis enlargement and thickening be performed at the same time?
Ideally, penis thickening is performed in combination with penis lengthening in order to achieve a satisfactory result. A tip-of-penis thickening (glans penis enlargement) can also be performed. This is also a common approach – more than 90% of men opt for the combined procedure.
Can a penis head (glans) thickening be performed at the same time as a penis enlargement?
Yes. A desired tip-of-penis enlargement can be performed at the same time as penis lengthening/penile thickening. However, it can also be done later at any time.
Will I keep my full erectile function after a penis enlargement?
With the surgical method I use, which involves detaching the anterior retaining ligaments, I work far away from the penis shaft and the regions that are important for an erection are not affected. The likelihood of erectile dysfunction due to penis enlargement is therefore very low.
How many centimeters can I lengthen my penis with surgery?
The result of a surgical penis enlargement is very individual, as the increase in length depends on three factors:
- The degree of inclination of the penis part that is brought from the inside to the outside and thereby straightened
- From the length of the connecting cartilage with the pelvic bone
- From the nature of your ligaments.
In most cases, it can be assumed that an extension of 2cm – 5cm can be achieved but in rare cases it is less.
What increase in girth can I achieve with a penis enlargement?
Using the autologous fat transfer method, up to 20% more penis circumference can be achieved.
What anesthesia is used for penis lengthening or penis thickening?
Penis lengthening and/or penis thickening is always performed on an outpatient basis under general anesthesia. You are allowed to go home again on the same day.
What do I need to consider after penis lengthening and/or penis thickening surgery?
The result of a surgical penis enlargement depends to a large extent on compliance with the instructions for the time after the operation.
The following points must be observed after penis lengthening/penile thickening in order not to jeopardize the surgical result:
Rest during a six-week healing phase in which the following things are not permitted:
- Refrain from sexual intercourse and masturbation for at least 6 weeks
- Avoid pulling, rubbing or pressure on the penis
- Full baths for at least 6 weeks
- Sauna for 6 months
- Sport for at least 6 weeks
In addition, the bandage applied after the operation must be worn as prescribed by the doctor and regular stretching exercises for the penis must be carried out. This prevents the penis from retracting again. A special vacuum pump should be used from 6-8 weeks after the operation.
For your own safety, always comply with agreed checks.
Are there other surgical methods of penis enlargement?
Yes, there are, e.g. the use of rods or other implants, but due to the poor results I only use the method by detaching the anterior retaining bands.
How do I prepare for penis enlargement or penis thickening?
- Detailed consultation with your plastic and aesthetic surgeon.
- Take care of your body – a good general state of health is also required for penis enlargement surgery.
- If you suffer from diabetes or high blood pressure, your doctor must adjust your medication optimally.
- If you are taking blood-thinning medication, discontinue it in consultation with your doctor or take a suitable substitute medication.
- Five days before and after the operation, you must not take any aspirin-containing or other anti-inflammatory drugs, as these have a massive effect on blood clotting, and herbal remedies and dietary supplements also fall into this category.
- If you fall ill with a feverish infection or herpes, please let us know immediately, as in this case the operation will have to be postponed.
- You should refrain from smoking for six weeks before and after the operation in order to minimize the risk of circulatory and wound healing disorders caused by smoking.
- Prepare everything you need at home after the operation and organize help if necessary so that you can relax and recover
- Excessive alcohol consumption should be avoided before and after the operation.
- Make sure you drink enough water to support the healing process.
What are the risks of penis lengthening or penis thickening?
No operation is free of risks – therefore, despite adherence to all precautionary measures, the most modern surgical methods and the best technical equipment, there are still certain risks involved in surgical penis lengthening or penis thickening. All of the risks and complications listed are generally very rare. However, in order to support you in your freedom of choice, I will list them all in full.

- Wound healing disorders (can be demanding and time consuming)

As with any operation, there are also general surgical risks such as intolerance to medication, anesthetics or bandages. If you are aware of an intolerance, please inform us in good time before the procedure.
Does health insurance pay for penis lengthening or penis thickening?
Penis lengthening and penis thickening are not covered by health insurance. The only exception may be the existence of a micropenis, which the health insurance company can approve at its discretion. Please contact your health insurance company.
When can I do sports and go to the sauna again after a penis enlargement?
Around six weeks after a penis enlargement, you can slowly start doing sports again, although heavy physical exertion should be avoided at first. Six months after the procedure, you may also return to the sauna.
