Size and appearance of the male penis
The size and appearance of the penis are very important for many men. Even if it is repeatedly asserted that the size of the penis has nothing to do with sexual performance, most men still attach importance to conforming to a certain standard in this area.
This goes hand in hand with a high degree of self-confidence, self-acceptance and sometimes also the ability to lead a fulfilling love life. If the psychological stress is very high, serious sexual health disorders such as erectile dysfunction and others can occur.
Penis thickening is usually performed in combination with penis enlargement, but can also be performed as a separate procedure.
Penis enlargement and penis thickening are always performed on an outpatient basis under general anesthesia. You are allowed to go home again on the same day.
Penis enlargement at a glance
Penis lengthening is the surgical displacement of the part of the penis located inside the body to the outside in order to lengthen the penis by 2-5 cm in most cases.
Penis enlargement method
The ratio of the inner and outer parts of the
penis is approx. 50:50
Your penis consists of a visible, external part and an internal section, whereby the ratio is approximately 50:50. Inside the body, the penis is attached to the pelvic bone with anterior and posterior ligaments.
During a penis enlargement, the inner part of the penis is moved outwards. To do this, an angled incision is made at the base of the penis in the pubic hair area to detach the anterior retaining ligaments from the pelvic bone. This straightens the previously curved and sloping part of the penis, which is a significant part of the lengthening procedure. Stability is maintained as the posterior retaining ligaments remain unchanged and any gaps are filled with the patient’s own tissue.
The elongated part of the penis is covered with the existing skin of the penis and the pubic mound. The condition of the ligaments and the length of the cartilage connecting to the pelvic bone are additional factors that influence the achievable lengthening. Finally, a bandage is applied, which must be worn according to the post-operative instructions to ensure an optimal result.
Achievable penis enlargement
The achievable length gain, i.e. how many centimeters a penis can be lengthened, depends on the existing inner penis length, the length and tightness of the ligament connecting the penis to the pelvic bone and the condition of the ligament.
Depending on the anatomical conditions, a length gain of around 2 – 4 cm can be achieved in most cases, in favorable cases even 5 cm. Rarely only a small amount of length can be gained due to anatomical preconditions. Unfortunately, this cannot be predicted or determined preoperatively.
Alternative surgical approaches to penis enlargement
There are alternative approaches to surgical penis enlargement, e.g. the use of rods or other implants. Due to the poor results, I only use the method of detaching the anterior retaining ligaments.
Penis thickening at a glance
Penis thickening with autologous fat can increase the girth of a thin penis by up to 20%. Penis thickening is usually performed at the same time as penis enlargement. The patient’s own fat is used as a filling material, as it does not cuae any defense reactions and around 50%-70% of the fat grows in permanently.
Method of penis thickening / girth enhancement
Based on my experience and success, I only use the autologous fat method to thicken the shaft of the penis. This is done in two steps:
First, the patient’s own fat is removed by liposuction, for example from the abdomen or thighs. The extracted fat is specially purified and prepared for transfer to the penis shaft.
The prepared autologous fat is then injected evenly under the skin of the penis shaft using a special technique and microcannulas. As not all of the injected autologous fat remains permanently, a little more is injected to achieve the desired result.
Experience has shown that around 50%-70% of the autologous fat introduced grows over the following weeks and months and is permanently retained. This value depends on individual physical factors and can vary from case to case.
Immediately after the operation, a special pressure bandage is applied, which you should wear for 4 weeks to support the best possible result. If desired, additional autologous fat can be injected in a second session to optimize the result.

3 steps to penis lengthening and thickening/girth enhancement
Anesthesia
Surgical penis enlargement or penis thickening with autologous fat is always performed under anesthesia. This is tailored to your personal well-being by our experienced anesthetist so that you can go home on the same day.
Surgical penis enlargement and / or penis thickening
The procedure usually takes one to two hours. First, the surgical area is carefully prepared to ensure that the procedure is performed safely and precisely.
During penis enlargement, a targeted incision is made at the base of the penis in order to move the inner part of the penis outwards. The anterior retaining ligaments are carefully released while the posterior ones provide stability.
Penis thickening with autologous fat is performed in parallel or as a separate procedure. The previously prepared fat is injected under sterile conditions and evenly distributed to ensure a natural shape.
Once the procedure is complete, a protective dressing is applied. The patient is discharged on the same day, accompanied by detailed instructions on aftercare and recovery support.
Aftercare and recovery
HEALING after penile thickening/girth enhancement: The healing process takes about 3 months in total. Immediately after the operation, a specially wrapped bandage is applied, which must be worn for 4 weeks. After 4 days, the bandage can be removed from the penis for the first time and light massages of the penis can be performed. You will receive special instructions for this after the operation.
During the healing phase, a "rest period" of at least 6 weeks must be observed.
This applies to sexual intercourse as well as masturbation and anything that causes tension, friction or pressure.
You will be fit for work again after around seven days and light sporting activity is permitted after around six weeks. After the healing phase of 6-8 weeks, regular postoperative stretching exercises must be performed so that the penis does not retract again. Special vacuum pump systems are available for this purpose and we will be happy to advise you.

Ideally, penis thickening is performed in combination with penis lengthening in order to achieve a satisfactory result. A tip-of-penis thickening (glans penis enlargement) can also be performed. This is also a common approach – more than 90% of men opt for the combined procedure.
Can a penis head (glans) thickening be performed at the same time as a penis enlargement?
Yes. A desired tip-of-penis enlargement can be performed at the same time as penis lengthening/penile thickening. However, it can also be done later at any time.
How many centimeters can I lengthen my penis with surgery?
The result of a surgical penis enlargement is very individual, as the increase in length depends on three factors:
- The degree of inclination of the penis part that is brought from the inside to the outside and thereby straightened
- From the length of the connecting cartilage with the pelvic bone
- From the nature of your ligaments.
In most cases, it can be assumed that an extension of 2cm – 5cm can be achieved but in rare cases it is less.
What girth can I achieve with a penis enlargement?
Using the autologous fat transfer method, up to 20% more penis circumference can be achieved. If the increase in circumference is not sufficient, a follow-up treatment (touch-up) can be carried out after the healing phase.
What do I have to consider after a penis enlargement / penis thickening?
The result of a surgical penis enlargement depends to a large extent on compliance with the instructions for the time after the operation.
The following points must be strictly adhered to after a penis enlargement/penis thickening in order not to jeopardize the surgical result:
Rest during a six-week healing phase in which the following things are not allowed:
- Refrain from sexual intercourse and masturbation for at least 6 weeks
- Avoid pulling, rubbing or pressure on the penis
- Full baths for at least 6 weeks
- Sauna for 6 months
- Sport for at least 6 weeks
In addition, the bandage applied after the operation must be worn as prescribed by the doctor and regular stretching exercises for the penis must be carried out. This prevents the penis from retracting again. A special vacuum pump should be used from 6-8 weeks after the operation.
For your own safety, always comply with agreed checks.
How do I prepare for penis enlargement or penis thickening?
- Detailed consultation with your plastic and aesthetic surgeon.
- Take care of your body – a good general state of health is also required for penis enlargement surgery.
- If you suffer from diabetes or high blood pressure, your doctor must adjust your medication optimally.
- If you are taking blood-thinning medication, discontinue it in consultation with your doctor or take a suitable substitute medication.
- Five days before and after the operation, you must not take any aspirin-containing or other anti-inflammatory drugs, as these have a massive effect on blood clotting, and herbal remedies and dietary supplements also fall into this category.
- If you fall ill with a feverish infection or herpes, please let us know immediately, as in this case the operation will have to be postponed.
- You should refrain from smoking for six weeks before and after the operation in order to minimize the risk of circulatory and wound healing disorders caused by smoking.
- Prepare everything you need at home after the operation and organize help if necessary so that you can relax and recover
- Excessive alcohol consumption should be avoided before and after the operation.
- Make sure you drink enough water to support the healing process.
What are the risks of penis enlargement?
No operation is free of risks – therefore, despite adherence to all precautionary measures, the most modern surgical methods and the best technical equipment, there are still certain risks involved in surgical penis lengthening or penis thickening. All of the risks and complications listed are generally very rare. However, in order to support you in your freedom of choice, I will list them all in full.

Around six weeks after a penis enlargement, you can slowly start doing sports again, although heavy physical exertion should be avoided at first. Six months after the procedure, you may also return to the sauna.
